IQHealth is a personal view of your health record that allows you to:

      • communicate with your physician

      • manage appointments

      • access your medical record

      • request medication refills

      • send messages to your doctor

      • view your lab results

How do you sign up?

You will need a valid email address to receive the invitation.

Get connected with your medical information by joining the Patient Portal. Just follow these easy steps:

    1. Inform the staff you want to get connected to the patient portal. (They have a form for you that is also attached at the bottom of the page for you to print out.)

    2. Fill out the Get Connected Form found at the bottom of this web page.

    3. Give the form to one of your Heath Care Team.

    4. You will be sent an email with the invitation to activate your account.

    5. Click on the link and follow the steps outlined on the page.

    6. Don't forget to complete this process soon because the email invitation will expire after 90 days. We will send you a reminder before it expires. However, if you forget, just contact the office to receive a new email invitation.

Is your information secure?

Yes. IQHealth is HIPPA compliant and provides you with the ability to securely view, store, and share your health information.

Why is Cerner referenced on the website?

Cerner is the company we use to securely manage your electronic health record. IQHealth is a customized Cerner system which provides you with the medical record.

Your privacy is important to us. We are committed to protecting the privacy and security of our patients. This Medical Home will respect the rights of patients 18 years and older to privacy regarding their health information and records. Providers will meet with and examine these patients privately unless the patient requests that the parent or other person be present.
